Chronic Prostatitis VA Rating
38 CFR § 4.115b, Diagnostic Code 7527
Overview
Chronic prostatitis covers both bacterial and chronic pelvic pain syndrome — recurrent prostate inflammation causing urinary symptoms, perineal pain, and sometimes sexual dysfunction.
DC 7527 directs VA to rate the disability by analogy to voiding dysfunction or urinary tract infection criteria, depending on the predominant symptoms.
Erectile dysfunction is a common compensable secondary — eligible for SMC-K when loss of use is established.
How to Establish Service Connection
- Direct: documented in-service prostatitis episodes that became chronic.
- Secondary: prostatitis aggravated by service-connected diabetes (recurrent UTI risk) or as a residual of pelvic radiation.

| Rating | Criteria |
|---|---|
| 10% | Long-term drug therapy, 1–2 hospitalizations per year, and/or requiring intermittent intensive management (UTI criteria). |
| 20% | Daytime voiding interval less than one hour, OR awakening to void five or more times per night. |
| 30% | Recurrent symptomatic infection requiring drainage / frequent hospitalization (greater than two times/year), and/or requiring continuous intensive management. |
| 40% | Wearing absorbent materials which must be changed 2 to 4 times per day (urine leakage). |
| 60% | Use of an appliance or wearing of absorbent materials which must be changed more than 4 times per day. |
