- Name the document, not the topic: "C&P examination report for lumbar spine dated 12 June 2025," not "my back records."
- Give a date range when the exact date is unknown.
- Add: "If any portion is withheld, identify the exemption and release all reasonably segregable portions."
- You may run a targeted request and a full-file request at the same time; they are worked separately.
- Highest-value single documents: the rating codesheet, the C&P exam report or DBQ, the medical opinion relied on, VA Form 21-4192 employer responses, and deferred rating sheets.
